Abstract

This instrument is intended to facilitate the cleaning of a cavity, for example a bone site, in particular when it is a deep cavity penetrating longitudinally into a long bone. It comprises two coaxial tubes (10, 11) which form between them an annular passage for injection of cleaning liquid. The front end of the outer tube (11) is pierced with a series of radial holes (17). The central tubes (10) is connected at its rear end to a suitable reduced-pressure source. The liquid is introduced through a lateral tube (16) into the annular passage (12), whence it is ejected through the orifices (17), and the front end of the tube in order to entrain the bone fragments and debris into the central suction tube.

Description

(54) SURGICAL SITE CLEANSING APPARATUS
(71) We, NATIONAL RESEARCH
DEVELOPMENT CORPORATION, a
British Corporation established by Statute, of I(ingsgate House, 66-74 Victoria Street,
London, S.W. 1, do hereby declare the invention for which we pray that a patent may be granted to us, and the method by which it is to be performed, to be particularly described in and by the following statement:
This invention concerns surgical site cleansing apparatus and more particularly such instruments for use in the preparation of a bone site prior to securement of a prosthetic bone joint replacement component thereto.
It is now routine practice for the components of prosthetic bone joint replacements to be secured to bones, after suitable shaping of the bones to receive the components, in association with a gap-filling medium commonly referred to as bone cement. However, it has been shown that weaknesses can arise from this mode of securement if artefacts such as bone debris are not removed from the site after the shaping.
An object of the present invention is to facilitate the consequentially desired removal of artefacts from a bone site, and particularly when the site involves an elongated recess longitudinally penetrating the bone.
To this end the present invention provides a surgical site cleansing apparatus comprising: an instrument including a pair of tubular members connected in mutually co-extending and substantially coaxial manner to define a central passageway through the inner one of said members and an annular passageway through the instrument between said members, said members being open at a first pair of corresponding ends, and said outer member having a band of orifices around its circumference adjacent its first end; a vacuum source connected to the second end of said inner surface; and a source of pressurised cleansing liquid connected to the second end of said outer member adjacent said second end and in communication with said annular passageway.
In use of the proposed apparatus, the first end of the instrument is applied to the bone site, and the two sources are activated. This results in flow of liquid through the annular passageway to provide a longitudinally directed annular spray of liquid from the first end of the instrument and also a laterally directed spray formed by an array of radial jets from the orifices, which cleanse the bone site and flush artefacts therefrom to be drawn, with the sprayed liquid, through the central passageway.
One embodiment of the instrument of the proposed apparatus is illustrated, by way of example, in a longitudinal sectional view in the accompanying drawing.
The illustrated instrument comprises two tubular metal members 10 and 11 having circular cross-section and of which one, 10, is longer than and located coaxially within the other, with one end of the inner member projecting beyond the corresponding one end of the outer member, and with the other ends of the members aligned. The external diameter of the inner member is slightly less than the internal diameter of the outer member over a major part of the former from said other ends to define an annular space 12 therebetween, the remaining length of the inner member being enlarged to slidingly engage the same in said one end of the outer tube. This engagement is maintained by the provision of external threading 13 around said one end of the outer member and a co-operating nut 14 inwardly flanged to slidingly engage the corresponding end portion of the inner member. The nut 14 also secures a washer 15 between its flange and the adjacent end of the outer member to seal the connection between the members.
A further tubular metal member 16 is connected to the outer member adjacent the one end of the latter to communicate with the annular space, the member being inclined over the nut 14.
Adjacent the other, aligned ends, the outer member is formed around its circumference with a plurality of orifices 17, and the inner member is reduced in diameter over a short length thereof to enlarge the portion 18 of the annular space 12 adjacent to the orifices 17.
The illustrated device is intended to be used as described above, with cleansing liquid supplied to the free end of member 16, and with suction being applied to the space 19 within the inner member by way of the one end of such member. The enlarged annular space portion 18 serves as a local reservoir for pressurised fluid adjacent the outlets from which the liquid is sprayed to ensure even delivery to these outlets. It is presently preferred to supply liquid by use of a large disposable syringe and the free end of member 16 is formed with an internal taper 19 to facilitate receipt of the syringe nozzle, while suction is suitably applied by way of a flexible hose and the one end of member 10 can be formed externally with an outward bulge 20 to receive a suitably dimensioned hose in a tight fit therearound.
WHAT WE CLAIM IS:
1. Surgical site cleaning apparatus comprising: an instrument including a pair of tubular members connected in mutually co-extending and substantially coaxial manner to define a central passageway through the inner one of said members and an annular passageway through the instrument between said members, said members being open at a first pair of corresponding ends, and said outer member having a band of orifices around its circumference adjacent its first end; a vacuum source connected to the second end of said inner member; and a source of pressurised cleansing liquid connected to said outer member adjacent said second end and in communication with said annular passageway.
2. Apparatus according to Claim 1 wherein said annular passageway is radially enlarged adjacent said orifices.
3. Apparatus according to Claim 2 wherein said enlargement is produced by radial reduction of said inner member.
4. Apparatus according to Claim 1, 2 or 3 comprising a further-tubular member branching from said outer member and internally tapered at its free end for receipt of said liquid source therein to supply liquid to said annular passageway.
5. Apparatus according to claim 1 including an instrument substantially as herein described with reference to the accompanying drawing.

Claims (5)

Adjacent the other, aligned ends, the outer member is formed around its circumference with a plurality of orifices 17, and the inner member is reduced in diameter over a short length thereof to enlarge the portion 18 of the annular space 12 adjacent to the orifices 17. The illustrated device is intended to be used as described above, with cleansing liquid supplied to the free end of member 16, and with suction being applied to the space 19 within the inner member by way of the one end of such member. The enlarged annular space portion 18 serves as a local reservoir for pressurised fluid adjacent the outlets from which the liquid is sprayed to ensure even delivery to these outlets. 2. Apparatus according to Claim 1 wherein said annular passageway is radially enlarged adjacent said orifices.

4. Apparatus according to Claim 1, 2 or 3 comprising a further-tubular member branching from said outer member and internally tapered at its free end for receipt of said liquid source therein to supply liquid to said annular passageway.
