SPECIFICATION
Surgical dressings
The present invention relates to surgical dressings.
There are already many surgical dressings which are known and which have been successfully marketed. These known dressings are normally sterilisable and function to protect a wound and to provide a buffer against external impacts upon the wounded area.
It has been suggested that surgical dressings could include a bacteriocidal agent to combat any infection of the wound. However, the use of bacteriocides in dressings has proved inconvenient and, moreover, concern has been expressed that bacteriocides may interfere with the complex process of natural healing.
Nonetheless, there is a need for an effective healing aid for use in surgical dressings and a particular group of compounds which are ideal for this purpose has now been found. The compounds in question have proven bacteriostatic properties which they can exert in the vapour phase, thus obviating the need for direct contact between the compound and the wounded area. Still further, the said compounds have been shown to exert an excellent bacteriostatic effect when used in dressings and applied directly on to the wounded area.
Accordingly, the present invention comprises a sterilisable surgical dressing comprising (A) a water-insoluble absorbent backing and, adhered thereto, using an adherent means, a water-insoluble, perforated, pliable film of a synthetic polymer, there being incorporated into one or more of the backing, the film and the adherent means, from 0.01 to 5% by weight, based on the total dressing, of a compound of formula:
wherein X1 and X3 are the same or different and each represents a halogen atom, and X2 is hydrogen or a halogen atom; or (B) a water-insoluble nonabsorbent backing and an absorbent pad, there being incorporated into the pad from 0.01 to 5% by weight, based on the total dressing, of a compound of formula I.
Specific examples of compounds of formula I are:
3-, 4,4′ – trichloro – 2 – hydroxydiphenyl ether 4,4′ – dichloro – 2 – hydroxydiphenyl ether 4 – chloro – 4′ – bromo – 2 – hydroxydiphenyl ether
4 – chloro – 4′ – iodo – 2 – hydroxydiphenyl ether 4 – chloro – 4′ – fluoro – 2 – hydroxydiphenyl ether
4 – bromo – 4′ – chloro – 2 – hydroxydiphenyl ether
4 – bromo – 4′ – chloro – 2 – hydroxydiphenyl ether
4 – bromo – 2′, 4′ – dichloro – 2 – hydroxydiphenyl ether
4, 4′ – dibromo – 2 – hydroxydiphenyl ether and, especially,
2′, 4,4′ – trichloro – 2 – hydroxydiphenyl ether
The water-insoluble backings for the new surgical dressings are preferably capillary, sheeted, nonwoven, fibrous absorbent materials referred to in
British Patent Specification No.778,813, e.g. the product marketed as «Cellucotton». Preferred water-insoluble film components and adherent compounds are likewise those described at length in Specification 778,813, e.g. «Mylar» film or films of vinyl acetate-crotonic acid plasticised with dibutyl phthalate; and polyvinyl acetate/dibutyl phthalate cements.
The non-absorbent backings may be of conventional type, e.g. an elasticated fabric backing, an opaque plastics backing or a transparent plastics backing. The plastics backing may be waterproof and/or perforated to facilitate aeration of the wounded area. The absorbent pad may be e.g. of lint or gauze.
Advantages of the dressings of the present invention are e.g.:
a) that in the case of a severe wound containing foreign bodies, painful cleaning of the wound can be avoided since the bacteriostatic agent in the dressing successfully controls the bacteria in the wound; and
b) even in the case of sterile wounds e.g. burns, the bacterial situation can be controlled by the use of the new dressings.
The following Examples further illustrate the present invention.
Example 1
A sterile surgical dressing sold under the Trade
Name «Melolin» was modified by incorporating into:
(a) the absorbent backing
(b) the polymer film, or
(c) the cement (or the plasticizer component thereof) adhering the backing to the film, 0.1% by weight, based on the total dressing, of 2′, 4, 4′ – trichloro – 2 – hydroxy diphenyl ether (Irgasan DP 300).
The dressings proved very effective in controlling the development of bacteria present in wounds covered by the dressings.
Example 2
Samples of «Band Aid» plasters were impregnated with 0.1% of 2′, 4,4′ – trichloro – 2 – hydroxydiphenyl ether (IRGASAN DP 300) in the lint padding. The lint padding portions of the plasters so treated were then
placed on agar plates seeded with pure cultures of the bacteria Staphylococcus aureus A TCC 6538,
Escherichia coliNCTC8196, Proteus vulgaris NCIB 4175 and Salmonella typhiNCTC 786. «Band Aid»
plasters containing no IRGASAN DP 300 were used
as controls. The plates were incubated at 37″C for 18
hours and then examined for zones of growth inhibition around the plaster padding. The results are set
out in the following Table:

+ = no reduction in growth + = slight reduction in growth
– = complete suppression of growth
The results obtained clearly demonstrate that the plasters containing 0.1% of IRGASAN DP 300 showed an excellent bacteriostatic effect against both Gram-positive and Gram-negative organisms, representative of types likely to be associated with wound infection. In contrast, the control samples showed a much lower level of stasis.
Claims (10)
1. Asterilisablesurgical dressing comprising (A) a water-insoluble absorbent backing and, adhered thereto using an adherent means, a water-insoluble, perforated, pliable film of a synthetic polymer, these being incorporated into one or more of the backing, the film and the adherent means, from 0.01 to 5% by weight, based on the total dressing, of a compound of formula:
wherein X, and X3 are the same or different and each represents a halogen atom, and X2 is hydrogen or a halogen atom; or (B) a water-insoluble, nonabsorbent backing and an absorbent pad, there being incorporated into the pad from 0.01 to 5% by weight, based on the total dressing, of a compound of formula I.
2. A dressing as claimed in Claim 1, wherein the compound of formula I is 2′, 4,4′ – trichloro – 2 hydroxydiphenyl ether.
3. A dressing as claimed in Claim 1 or 2, wherein the water-insoluble backing is a capillary, sheeted, non-woven, fibrous, absorbent material described in
British Patent Specification No.778,813.
4. A dressing as claimed in any of the preceding claims, wherein the water-insoluble film and the adherent means are those described in British Patent
Specification No.778,813.
5. A dressing as claimed in Claim 4, wherein the water-insoluble film is a «Mylar» film or is a film of vinyl acetate-crotonic acid plasticised with dibutyl phthalate.
6. A dressing as claimed in Claim 4 or 5, wherein the adherent means is a polyvinyl acetate/dibutyl
phthalate cement.
7. A dressing as claimed in Claim 1 or 2, wherein the non-absorbent backing is an elasticated fabric
backing, or an opaque- or transparent plastics backing.
8. A dressing as claimed in Claim 7, wherein the plastics backing is waterproof and/or perforated to facilitate aeration of the wound.
9. A dressing as claimed in any of Claims 1,2,7 and 8, wherein the absorbent pad is of lint or gauze.
10. A sterilisable surgical dressing substantially as described with reference to either of the Examples.
